[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide fully-insured health, dental, and vision plans, Medicare, the cost of wellness plan administration and COBRA administration with ability to provide regular claims and utilization reporting to the city including monthly claims usage, a rolling average of claims, utilization reporting, and quarterly high claims reports for claims over $25,000 (to include status of claim, diagnosis, relationship to claimant, and cost).
(2) All questions must be submitted no later than April 27, 2018.
(3) Contract period will be one year.
[C] Eligibility:
- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location.
